ANATOMATICAL CONSIDERATIONS IN LENS CHOICE Sometimes the patients ocular anatomy dictates the lens choice. If the patient has a distorted cornea the only lens type that may give acceptable vision is a rigid lens. Since the rigid lens masks the distortion by allowing the tear layer to neutralize it, the front of the lens becomes the new, regular front refracting surface of the eye. A hydrogel lens will conform to the irregular surface with no significant tear layer to neutralize the distortion, therefore, the vision is not significantly improved. Patients with corneal distortion from trauma, disease, surgery, keratoconus or other causes usually are best fit with rigid lenses.
